ਅਤਿ ਜਰੂਰੀ ਸੂਚਨਾ
ਵਿਦਿਆਰਥੀਆਂ ਨੂੰ ਸੂਚਿਤ ਕੀਤਾ ਜਾਂਦਾ ਹੈ ਕਿ ਉਹ ਡਿਸਟੈਂਸ ਐਜੂਕੇਸ਼ਨ ਵਿਭਾਗ ਵਿੱਚ ਦਾਖ਼ਲਾ ਲੈਣ ਲਈ ਪੂਰਾ ਪ੍ਰਾਸਪੈਕਟਸ ਹੀ ਡਾਊਨਲੋਡ ਕਰਨ, ਨਾ ਕਿ ਸਿਰਫ਼ ਫਾਰਮ, ਕਿਉਂਕਿ ਪ੍ਰਾਸਪੈਕਟਸ ਵਿੱਚ ਦਿਤੀਆਂ ਹਦਾਇਤਾਂ ਨੂੰ ਪੜ੍ਹੇ ਬਿਨਾਂ ਦਾਖ਼ਲਾ ਫਾਰਮ, ਹੋਰ ਫਾਰਮ ਅਤੇ ਓ.ਐਮ.ਆਰ. ਸ਼ੀਟ ਦੇ ਵੇਰਵਿਆਂ ਨੂੰ ਭਰਨਾ ਅਸੰਭਵ ਹੈ|
ਵਿਦਿਆਰਥੀਆਂ ਲਈ ਹਿਦਾਇਤਾਂ
1) ਜੇਕਰ ਵਿਦਿਆਰਥੀ ਡਾਊਨਲੋਡ ਫਾਰਮ ਨੂੰ ਭਰ ਕੇ ਡਾਕ ਰਾਹੀਂ ਭੇਜਦੇ ਹਨ ਤਾਂ ਉਹ ਓ.ਐਮ.ਆਰ. ਸ਼ੀਟ ਲਈ Excel Student Entry Sheet ਦੇ ਲਿੰਕ ਤੇ ਕਲਿਕ ਕਰਕੇ Excel ਦੀ ਫਾਈਲ ਡਾਊਨਲੋਡ ਕਰਨ ਉਪਰੰਤ ਇਸਨੂੰ ਪੂਰੀ ਤਰ੍ਹਾਂ ਭਰਕੇ programmerde@yahoo.com ਤੇ ਈਮੇਲ (as an attachment) ਭੇਜਣਾ ਯਕੀਨੀ ਬਣਾਉਣ| Excel Student Entry Sheet ਦਾ ਪ੍ਰਿੰਟ ਫਾਰਮ ਨਾਲ ਨਹੀਂ ਭੇਜਣਾ, ਸਿਰਫ਼ ਈਮੇਲ ਹੀ ਕਰਨੀ ਹੈ|
2) ਜੇਕਰ ਵਿਦਿਆਰਥੀ ਡਾਊਨਲੋਡ ਫਾਰਮ ਨੂੰ ਦਸਤੀ (by hand) ਜਮ੍ਹਾਂ ਕਰਾਉਂਦੇ ਹਨ ਤਾਂ ਉਹ ਓ.ਐਮ.ਆਰ. ਸ਼ੀਟ ਯੂਨਿਵਰਸਿਟੀ ਦੇ ਮੇਨ ਗੇਟ ਤੇ ਸਥਿਤ ਪੁੱਛ-ਗਿੱਛ ਅਤੇ ਸੂਚਨਾ ਕੇਂਦਰ ਤੋਂ, ਡਾਊਨਲੋਡ ਕੀਤੇ ਫਾਰਮ ਅਤੇ ਕੋਰਸ ਫੀਸ ਜਮ੍ਹਾਂ 350/- ਰੁਪਏ ਪ੍ਰਾਸਪੈਕਟਸ ਦੀ ਕੀਮਤ ਦੇ ਡਰਾਫ਼ਟ ਦਾ ਸਬੂਤ ਦਿਖਾ ਕੇ (ਬਿਨਾਂ ਕਿਸੇ ਕੀਮਤ ਤੋਂ) ਪ੍ਰਾਪਤ ਕਰ ਸਕਦੇ ਹਨ| ਓ.ਐਮ.ਆਰ. ਸ਼ੀਟ ਨੂੰ ਭਰਨ ਉਪਰੰਤ ਬਾਕੀ ਫਾਰਮਾਂ ਨਾਲ ਜਮ੍ਹਾਂ ਕਰਵਾਉਣਾ ਲਾਜ਼ਮੀ ਹੈ|
3) ਇੱਥੇ ਇਹ ਵੀ ਦੱਸਿਆ ਜਾਂਦਾ ਹੈ ਕਿ ਵਿਦਿਆਰਥੀ ਡਾਊਨਲੋਡ ਕੀਤੇ ਦਾਖ਼ਲਾ ਫਾਰਮ ਨਾਲ ਸੰਬੰਧਤ ਜਿਹੜਾ ਕੋਰਸ ਕਰਨਾ ਚਾਹੁੰਦੇ ਹਨ, ਉਸ ਦੀ ਫੀਸ ਅਤੇ ਉਸ ਵਿੱਚ ਹੀ ਪ੍ਰਾਸਪੈਟਸ ਦੀ ਫੀਸ 350/- ਰੁਪਏ ਜਮ੍ਹਾਂ ਕਰਕੇ ਇੱਕੋ ਹੀ ਡਰਾਫਟ ਰਜਿਸਟਰਾਰ, ਪੰਜਾਬੀ ਯੂਨਿਵਰਸਿਟੀ ਪਟਿਆਲਾ ਦੇ ਨਾਂ ਤੇ ਬਣਵਾਉਣ| ਦਾਖ਼ਲਾ ਫਾਰਮ ਤੇ ਫੀਸ ਵਾਲੇ ਕਾਲਮ ਵਿੱਚ ਵੀ ਇਹ ਲਿਖ ਦੇਣ ਕਿ ਇਸ ਫੀਸ ਵਿੱਚ ਕੋਰਸ ਦੀ ਫੀਸ ਅਤੇ ਪ੍ਰਾਸਪੈਟਸ ਦੀ ਕੀਮਤ ਸ਼ਾਮਿਲ ਹੈ|
Important Instructions
Students seeking admission in any course in the Department of Distance Education are advised to download the entire prospectus and not just the admission form. This is essential because the admission form, examination form, OMR sheet and other forms cannot be filled without reading the instructions given in the prospectus.
1) If the student wants to send the downloaded admission form via post, then for OMR sheet he/she must make sure to click the link Excel Student Entry Sheet and download the Excel file. This file must be filled completely and sent to programmerde@yahoo.com as Email attachment. This Excel Student Entry Sheet is not to be sent with other forms in print mode. The other forms can be sent via post.
2) If the student wants to submit the downloaded form by hand, then he/she must show the downloaded form and proof of demand draft (Rs. 350/- as cost of prospectus plus the course fee) at the Information and Enquiry Centre located at the main gate of the Punjabi University, Patiala and obtain the OMR sheet free of cost. The OMR sheet must then be filled and submitted alongwith the other forms only. No form will be entertained without OMR sheet.
3) It is further made clear that students who download the form must get a single demand draft made, which should include not only the course fee but also the cost of prospectus (Rs. 350/-). This draft should be made in favour of Registrar, Punjabi University, Patiala. In the admission form, the student should clearly indicate in the fee column that the amount filled in as fee includes the cost of prospectus also.
